Aliskiren is an orally active, non-peptide, renin inhibitor used to treat hypertension. It belongs to the class of direct renin inhibitors (DRIs) and works by blocking the action of renin, a key enzyme in the renin-angiotensin-aldosterone system (RAAS), which regulates blood pressure. By inhibiting renin, aliskiren reduces the production of angiotensin I and subsequently angiotensin II, leading to vasodilation and decreased blood pressure.
For the treatment of hypertension (high blood pressure).
Do not use aliskiren with ACE inhibitors or ARBs in patients with diabetes due to the risk of renal impairment, hypotension, and hyperkalemia.
Outcome:
Increased risk of renal impairment, hypotension, and hyperkalemia.
Mechanism:
Synergistic effects on the RAAS system.
Outcome:
Reduced antihypertensive effect of aliskiren.
Mechanism:
Unknown.
Outcome:
May decrease aliskiren absorption.
Mechanism:
Changes in gastric pH
Most likely new formulation: Combination therapy with other antihypertensives (2025, 70% confidence)
Based on current usage trends and clinical trial data, there is a low probability (<10%) of aliskiren being withdrawn from the market in the next 5 years.
