Bosentan is a dual endothelin receptor antagonist used to treat pulmonary arterial hypertension (PAH). It works by blocking the effects of endothelin, a peptide that causes blood vessels to constrict, thereby improving blood flow and reducing pulmonary vascular resistance.

Bosentan can cause severe liver injury, including liver failure and death. Liver function tests must be performed monthly. Bosentan is contraindicated in pregnancy due to its potential to cause birth defects.
