Brand: Trasylol
BPTI, also known as aprotinin, is a small protein that acts as a serine protease inhibitor. It's naturally found in bovine lungs and has been used clinically to reduce bleeding during complex surgeries, particularly cardiac surgery. It works by inhibiting several enzymes involved in the blood clotting cascade, including trypsin, chymotrypsin, kallikrein, and plasmin.
Used to reduce bleeding during and after surgery, especially cardiac surgery.
Outcome:
Increased risk of bleeding
Mechanism:
Synergistic anticoagulant effects
Outcome:
Increased risk of renal dysfunction
Mechanism:
Potential additive effects on renal function
Outcome:
Increased bleeding risk
Mechanism:
Additive antiplatelet effects
Research into more specific and safer protease inhibitors is ongoing, with new candidates potentially emerging in the next 5-10 years (70% confidence).
Due to safety concerns and withdrawal from many markets, the likelihood of BPTI being reintroduced for widespread clinical use is very low (<1%).
