Brand: Bretylol
Bretylium tosylate is a Class III antiarrhythmic agent that was primarily used to treat life-threatening ventricular arrhythmias, particularly ventricular fibrillation and ventricular tachycardia that did not respond to other medications. It acts by blocking the release of norepinephrine from nerve endings, prolonging the action potential duration, and increasing the ventricular fibrillation threshold. However, due to its side effects and the availability of newer, more effective antiarrhythmic drugs, bretylium is rarely used today.
Used for treating life-threatening ventricular arrhythmias.
Outcome:
Increased risk of digoxin toxicity
Mechanism:
Bretylium can enhance digoxin's effects on the heart.
Outcome:
Increased risk of hypotension
Mechanism:
Additive hypotensive effects.
Outcome:
Slightly decreased antihypertensive effect
Mechanism:
NSAIDs can reduce the effectiveness of some antihypertensives
Unlikely to see new formulations of bretylium due to its side effect profile and availability of safer alternatives.
Due to its withdrawal, there is a 0% likelihood of bretylium being reintroduced in its previous formulation.
