Brand: Marcaine, Sensorcaine, Bupivacaine Hydrochloride, Bupivacaine HCl
Bupivacaine is a local anesthetic belonging to the amide class. It blocks nerve conduction by inhibiting sodium influx through voltage-gated sodium channels in neuronal cell membranes. This action prevents the generation and conduction of nerve impulses, resulting in a reversible loss of sensation in the area where it is administered. Bupivacaine is widely used for various types of regional anesthesia, including epidural, spinal, peripheral nerve blocks, and local infiltration.
Bupivacaine is used for local anesthesia during surgical procedures, labor and delivery, and pain management.
Cardiac arrest can occur with accidental intravascular injection of bupivacaine intended for epidural, caudal, or peripheral nerve block administration.
Outcome:
Increased bupivacaine plasma levels, increased risk of toxicity
Mechanism:
Inhibition of bupivacaine metabolism
Outcome:
Increased risk of bradycardia and hypotension
Mechanism:
Additive cardiovascular effects
Outcome:
May slightly decrease bupivacaine absorption
Mechanism:
Altered gastrointestinal pH
Most likely new formulation: Liposomal bupivacaine for extended release pain control (2024, 70% confidence)
Based on current usage trends and clinical trial activity, there is a low ( <5%) likelihood of a major regulatory change regarding bupivacaine's approved indications in the next 2 years.
