Brand: Capastat
Capreomycin sulfate is an antimycobacterial drug used to treat tuberculosis, specifically multidrug-resistant strains. It is a cyclic polypeptide antibiotic produced by the bacterium Streptomyces capreolus. Capreomycin inhibits protein synthesis in susceptible mycobacteria, leading to bacterial cell death. It's generally used in combination with other anti-tuberculosis medications due to the potential for resistance development.
Used to treat multidrug-resistant tuberculosis (MDR-TB) in combination with other anti-TB medications.
Capreomycin can cause serious side effects, including hearing loss (ototoxicity), kidney damage (nephrotoxicity), and problems with balance. Patients should be monitored closely for these effects during treatment.
Outcome:
Increased risk of ototoxicity and nephrotoxicity
Mechanism:
Additive toxicity on the kidneys and ears
Outcome:
Decreased renal function
Mechanism:
NSAIDs can reduce renal blood flow, exacerbating capreomycin-induced nephrotoxicity
Outcome:
Reduced absorption of capreomycin
Mechanism:
Antacids can bind to capreomycin and decrease its absorption
Most likely new formulation: Liposomal capreomycin for improved delivery and reduced toxicity (2025, 60% confidence)
Based on current resistance patterns, there is a 20% likelihood of new capreomycin-resistant TB strains emerging within the next 5 years.
