Brand: Miostat, Isopto Carbachol
Carbachol is a cholinergic agonist that stimulates both muscarinic and nicotinic receptors. It is used primarily in ophthalmology to treat glaucoma and to induce miosis during surgery. Unlike acetylcholine, carbachol is not metabolized by cholinesterase, resulting in a longer duration of action.
Used to treat glaucoma and induce miosis during surgery.
Outcome:
Increased risk of cholinergic adverse effects (e.g., bradycardia, hypotension, bronchospasm).
Mechanism:
Additive cholinergic effects.
Outcome:
Increased risk of cardiac adverse effects.
Mechanism:
Additive effects on cardiac conduction.
Outcome:
Potential for increased intraocular pressure.
Mechanism:
NSAIDs can increase intraocular pressure.
Most likely new formulation: Sustained-release intraocular implant (Year: 2028, 70% confidence)
Based on current usage trends and ongoing research, there is a low (5%) likelihood of significant regulatory changes in the next 5 years.
