Colchicine is a medication primarily used to treat gout and familial Mediterranean fever (FMF). It belongs to a class of drugs known as anti-gout agents and works by reducing inflammation caused by uric acid crystals in the joints. Colchicine is derived from the autumn crocus plant and has been used medicinally for centuries. While effective, it has a narrow therapeutic index, meaning the effective dose is close to the toxic dose, requiring careful monitoring.
For treating gout flares, preventing gout attacks, and managing familial Mediterranean fever.
Serious, sometimes fatal, blood dyscrasias (e.g., agranulocytosis, aplastic anemia, thrombocytopenia) can occur with colchicine. Patients with renal or hepatic impairment, especially those receiving long-term treatment, are at increased risk. Monitor for early signs of toxicity, such as fever, sore throat, bleeding/bruising, and weakness. Fatal overdoses have been reported with colchicine.
Outcome:
Increased risk of colchicine toxicity
Mechanism:
Clarithromycin inhibits CYP3A4 and P-gp, reducing colchicine clearance.
Outcome:
Increased risk of colchicine toxicity
Mechanism:
Inhibition of CYP3A4 and P-gp.
Outcome:
Decreased colchicine absorption
Mechanism:
Altered GI pH or chelation.
Most likely new formulation: sustained-release colchicine (2025, 80% confidence)
Based on increasing off-label use and positive clinical trial results, there is a 70% likelihood of expanded indications for colchicine in the next 5 years.
