Dabigatran is an anticoagulant drug used to prevent and treat blood clots. It is a direct thrombin inhibitor, meaning it directly blocks the activity of thrombin, a key enzyme in the clotting process. Dabigatran is commonly prescribed to prevent strokes in patients with atrial fibrillation, a type of irregular heartbeat, and to treat and prevent deep vein thrombosis (DVT) and pulmonary embolism (PE). It is also increasingly being used after certain types of orthopedic surgeries, such as hip or knee replacement, to prevent blood clot formation.
For prevention of stroke and systemic embolism in patients with non-valvular atrial fibrillation, treatment and prevention of DVT and PE, and prevention of blood clots after orthopedic surgeries.
Spinal or epidural hematomas may occur in patients treated with dabigatran who undergo spinal procedures, including spinal puncture. These hematomas may result in long-term or permanent paralysis. Factors that can increase the risk of developing a spinal or epidural hematoma in these patients include the use of indwelling epidural catheters, concomitant use of other drugs that affect hemostasis (e.g., NSAIDs, platelet inhibitors, other anticoagulants), and traumatic or repeated epidural or spinal punctures. Monitor patients frequently for signs and symptoms of neurological impairment. If neurological compromise is noted, urgent treatment is required.
