Hetastarch is a synthetic colloid used as a plasma volume expander in the treatment of hypovolemia (low blood volume). It belongs to the class of hydroxyethyl starches, which are derived from amylopectin. Hetastarch works by increasing the osmotic pressure in the blood vessels, drawing fluid from the interstitial space into the bloodstream. This helps to restore blood volume and improve circulation.
Used to treat hypovolemia (low blood volume) due to various causes like bleeding, surgery, or trauma.
WARNING: Increased risk of death and severe kidney injury. Hetastarch is contraindicated in critically ill adults, including those with sepsis and those undergoing open heart surgery with cardiopulmonary bypass, and in patients with pre-existing kidney injury. Avoid use in patients with severe bleeding.
Research on safer plasma expanders is ongoing, potentially leading to new alternatives within the next decade (2030-2040, 70% confidence).
Due to safety concerns and withdrawal from many markets, the likelihood of hetastarch regaining widespread use is low (<5%).
