Methysergide is a semisynthetic ergot alkaloid used to prevent migraine and cluster headaches. It acts as a serotonin receptor antagonist, specifically targeting the 5-HT2A and 5-HT2C receptors. While effective, it carries the risk of serious side effects, including fibrosis, which limits its long-term use.

Methysergide may cause fibrosis of retroperitoneal, pleuropulmonary, cardiac, and other tissues. Patients must be monitored for symptoms such as chest pain, abdominal pain, dyspnea, and edema. Therapy should be interrupted if these symptoms appear. Methysergide maleate should not be used for the treatment of migraine or cluster headaches continuously for longer than six months. A drug-free interval of three to four weeks should follow each six-month course of treatment.
