N-carbamylglutamate (NCG) is a structural analog of N-acetylglutamate (NAG), an essential allosteric activator of carbamoyl phosphate synthetase I (CPSI), the first and rate-limiting enzyme of the urea cycle. NCG is used to treat hyperammonemia, especially in those with urea cycle disorders (UCDs). It helps stimulate the urea cycle to eliminate excess ammonia. It is classified as an orphan drug.
Used to treat hyperammonemia, particularly in patients with urea cycle disorders.
Outcome:
Increased risk of hyperammonemia
Mechanism:
Inhibition of NAGS activity
Outcome:
Additive effect on ammonia reduction
Mechanism:
Different mechanisms of action leading to synergistic effect.
Outcome:
Alterations in gut flora can affect ammonia production.
Mechanism:
Changes in gut bacteria that produce ammonia.
Most likely new formulation: Extended-release NCG (Year 2026, 80% confidence)
Based on current clinical trial data and usage trends, there is a 70% likelihood of expanded indications for NCG in other metabolic disorders within the next 5 years.
