Nalidixic acid is a first-generation quinolone antibiotic primarily used to treat urinary tract infections (UTIs). It works by inhibiting bacterial DNA gyrase, an enzyme essential for bacterial DNA replication and repair. While effective against many gram-negative bacteria causing UTIs, it has limited effectiveness against other infections and is no longer a first-line treatment due to the development of more effective and safer antibiotics.
For the treatment of uncomplicated urinary tract infections caused by susceptible gram-negative bacteria.
Outcome:
Increased risk of bleeding
Mechanism:
Nalidixic acid can displace warfarin from plasma proteins, increasing its free concentration.
Outcome:
Increased risk of seizures
Mechanism:
Both NSAIDs and nalidixic acid can lower the seizure threshold.
Outcome:
Delayed absorption of nalidixic acid
Mechanism:
Food can slow the rate of absorption but not the extent.
Most likely new development related to nalidixic acid: research exploring its potential use in combination therapy for drug-resistant bacteria (Year: 2025, 10% confidence).
Based on declining usage and the availability of safer alternatives, there is a low (5%) likelihood of new formulations or indications being developed for nalidixic acid.
