Perhexiline is an antianginal drug that belongs to the class of piperazine derivatives. It primarily works by inhibiting mitochondrial carnitine palmitoyltransferase-1 (CPT-1), thereby shifting myocardial metabolism from fatty acid oxidation to glucose oxidation. This metabolic shift is believed to improve myocardial efficiency and reduce angina symptoms in patients with coronary artery disease.
Used to treat angina pectoris (chest pain) when other treatments are ineffective.
Outcome:
Increased perhexiline levels, leading to toxicity.

Mechanism:
Inhibition of perhexiline metabolism.
Outcome:
May slightly decrease perhexiline absorption.
Mechanism:
Changes in gastric pH.
Most likely new formulation: Extended-release formulation to improve patient compliance and reduce side effects (Year: 2026, 30% confidence)
Due to the risk of serious side effects and the availability of safer alternatives, perhexiline is unlikely to be re-approved by the FDA in the near future (less than 5% likelihood within the next 5 years).
