Piroxicam is a nonsteroidal anti-inflammatory drug (NSAID) used to relieve pain, inflammation, and stiffness caused by conditions such as osteoarthritis, rheumatoid arthritis, and ankylosing spondylitis. It belongs to the oxicam class of NSAIDs and works by inhibiting the production of prostaglandins, substances that contribute to pain and inflammation.
For pain, fever, inflammation, and stiffness associated with osteoarthritis, rheumatoid arthritis, and ankylosing spondylitis.
Piroxicam, like other NSAIDs, may increase the risk of serious cardiovascular thrombotic events, myocardial infarction, and stroke, which can be fatal. This risk may increase with duration of use. Piroxicam is contraindicated in the setting of coronary artery bypass graft (CABG) surgery. Piroxicam, like other NSAIDs, can also increase the risk of serious gastrointestinal (GI) adverse events, including bleeding, ulceration, and perforation of the stomach or intestines, which can be fatal. These events can occur at any time during use and without warning symptoms. Elderly patients are at greater risk for serious gastrointestinal events.
Outcome:
Increased bleeding risk
Mechanism:
Piroxicam can displace warfarin from protein binding sites, increasing its free concentration.
Outcome:
Increased risk of GI bleeding
Mechanism:
Additive effects on GI irritation.
Outcome:
Reduced piroxicam absorption
Mechanism:
Antacids can interfere with piroxicam absorption.
Most likely new formulation: Topical piroxicam for localized pain relief (Year: 2025, 70% confidence)
Based on current usage trends and safety profile, there is a low (<5%) likelihood of piroxicam being withdrawn from the market in the next 5 years.
