Propylthiouracil (PTU) is an antithyroid drug used to treat hyperthyroidism, a condition characterized by an overactive thyroid gland. It belongs to the thionamide class of drugs and works by inhibiting the production of thyroid hormones. PTU is particularly useful in the first trimester of pregnancy and during thyroid storm due to its rapid action and lower risk of transfer to the fetus compared to other antithyroid medications.

Severe liver injury, including liver failure and death, has been reported with propylthiouracil. Liver injury may occur at any time during treatment, even after many months of therapy. Propylthiouracil-induced liver failure has resulted in the need for liver transplantation and death, particularly in children. Propylthiouracil should be reserved for use in patients who cannot tolerate other therapies, such as methimazole, radioactive iodine, or surgery. In adults, propylthiouracil is recommended only for patients in the first trimester of pregnancy or for patients experiencing thyroid storm.
