Prostacyclin is a potent vasodilator and inhibitor of platelet aggregation. It is a naturally occurring prostaglandin, specifically prostaglandin I2 (PGI2), produced by the endothelial cells lining blood vessels. It plays a crucial role in maintaining vascular homeostasis by preventing excessive vasoconstriction and thrombosis. Therapeutically, synthetic prostacyclin analogues are used for conditions requiring vasodilation, such as pulmonary arterial hypertension.
Used to treat pulmonary arterial hypertension, peripheral vascular disease, and prevent blood clots during certain medical procedures.
Outcome:
Increased risk of bleeding
Mechanism:
Synergistic effects on coagulation cascade
Outcome:
Enhanced hypotensive effects
Mechanism:
Synergistic vasodilatory effects
Outcome:
May reduce absorption of prostacyclin
Mechanism:
Altered gastrointestinal pH
Most likely new formulation: inhaled dry powder formulations and oral prostanoids (2025, 80% confidence)
Based on increasing prevalence of pulmonary arterial hypertension and positive clinical trial data for newer prostacyclin analogues, there is a 75% likelihood of new prostacyclin-related drug approvals in the next 5 years.
