Ramiprilat is the active metabolite of the prodrug ramipril. It is a long-acting angiotensin-converting enzyme (ACE) inhibitor used to treat hypertension (high blood pressure), heart failure, and to improve survival after a heart attack. It works by blocking the conversion of angiotensin I to angiotensin II, a potent vasoconstrictor. This leads to vasodilation and reduced blood pressure.
Used to treat hypertension, heart failure, and to improve survival after a heart attack.
Fetal toxicity: Use of drugs that act on the renin-angiotensin system during the second and third trimesters of pregnancy reduces fetal renal function and increases fetal and neonatal morbidity and death.
Outcome:
Hyperkalemia
Mechanism:
Reduced potassium excretion
Outcome:
Hypotension
Mechanism:
Additive hypotensive effects
Outcome:
Slightly reduced absorption
Mechanism:
Delayed gastric emptying
Most likely new formulation: Combination therapy with other antihypertensives (2025, 70% confidence)
Based on current usage trends and clinical trial data, there is a low likelihood of any major regulatory changes for ramiprilat in the next 5 years.
