Rivaroxaban is an anticoagulant medication (blood thinner) used to prevent and treat blood clots. It is a direct factor Xa inhibitor, belonging to the class of drugs known as DOACs (direct oral anticoagulants). Rivaroxaban works by blocking factor Xa, a key enzyme in the blood clotting cascade, thereby reducing the formation of thrombin and fibrin, essential components of a blood clot.
For prevention and treatment of deep vein thrombosis (DVT), pulmonary embolism (PE), stroke in patients with atrial fibrillation, and to reduce the risk of blood clots after hip or knee replacement surgery.
Spinal/epidural hematomas may occur with rivaroxaban, resulting in long-term or permanent paralysis. Risk is increased with the use of indwelling epidural catheters, concomitant NSAIDs or other antiplatelet therapy, traumatic or repeated epidural or spinal punctures, or a history of difficult or traumatic epidural or spinal puncture. Monitor for signs and symptoms of neurological impairment.
Outcome:
Increased bleeding risk
Mechanism:
Inhibition of rivaroxaban metabolism
Outcome:
Increased bleeding risk
Mechanism:
Additive effect on platelet function
Outcome:
Slightly decreased rivaroxaban absorption
Mechanism:
Chelation or altered pH
Most likely new formulation: Extended-release formulation for once-weekly dosing (Year 2026, 70% confidence)
Based on current usage trends and clinical trial data, there is a 90% likelihood that rivaroxaban will maintain its current FDA approval status for the next 5 years.
