Teniposide is a semisynthetic derivative of podophyllotoxin, classified as a topoisomerase II inhibitor. It interferes with DNA replication by inhibiting the enzyme topoisomerase II, which is essential for cell division. This leads to DNA strand breaks and ultimately cell death. Teniposide is primarily used in the treatment of certain types of cancer.
Used to treat refractory childhood acute lymphoblastic leukemia.
Myelosuppression (neutropenia, thrombocytopenia, anemia) is a significant side effect that can lead to severe infections and bleeding. Extravasation can cause severe local tissue damage.
Outcome:
Increased risk of neurotoxicity
Mechanism:
Synergistic effect on nerve damage
Outcome:
Decreased teniposide levels
Mechanism:
Induction of drug-metabolizing enzymes
Outcome:
Reduced teniposide absorption
Mechanism:
Changes in gastric pH
Most likely new formulation: Liposomal teniposide for improved delivery and reduced toxicity (Year: 2026, 30% confidence)
Based on current usage trends and clinical trial activity, there is a low likelihood of significant changes to teniposide's regulatory status in the next 5 years.
