Terbutaline is a beta-2 adrenergic agonist used primarily as a bronchodilator to treat asthma and other respiratory conditions like chronic obstructive pulmonary disease (COPD). It relaxes the smooth muscles in the airways, allowing them to open up and improve airflow. While also used to suppress premature labor, this use is associated with significant risks and is generally discouraged.
For treating asthma, chronic obstructive pulmonary disease (COPD), and other respiratory conditions; historically used to suppress premature labor, but now generally discouraged.
Terbutaline sulfate injection should not be used for the prevention or treatment of premature labor. Serious adverse reactions, including maternal death, have been reported with the use of terbutaline sulfate for tocolysis. When terbutaline sulfate injection is used for tocolysis, cases of pulmonary edema, cardiac arrhythmias, myocardial ischemia, and increased heart rate have been reported.
Outcome:
Reduced bronchodilator effect
Mechanism:
Pharmacodynamic antagonism
Outcome:
Increased risk of adverse effects (tremor, palpitations)
Mechanism:
Additive effects on the central nervous system and cardiovascular system
Outcome:
Increased risk of cardiovascular effects
Mechanism:
Additive effects on the cardiovascular system
Most likely new formulation: Long-acting inhaled terbutaline with improved safety profile (2025, 60% confidence)
Based on decreasing usage trends and safety concerns, there is a 70% likelihood of further restrictions on the use of terbutaline for tocolysis in the next 5 years.
