Topotecan is a semi-synthetic, water-soluble derivative of camptothecin, a natural product extracted from the bark and stem of the Chinese tree Camptotheca acuminata. It acts as a topoisomerase I inhibitor, preventing the re-ligation of single-strand DNA breaks created by the enzyme during DNA replication and transcription. This leads to DNA damage and ultimately cell death. Topotecan is primarily used to treat certain types of cancer, including ovarian cancer, small cell lung cancer, and cervical cancer.

Myelosuppression (neutropenia, thrombocytopenia, anemia). Interstitial lung disease (ILD), including fatal cases, has been reported. Topotecan should be discontinued in patients who develop new or progressive pulmonary symptoms.
