Treprostinil is a synthetic analog of prostacyclin (PGI2) used to treat pulmonary arterial hypertension (PAH). It is a vasodilator, meaning it widens blood vessels, which helps to lower blood pressure in the lungs. Treprostinil also inhibits platelet aggregation, reducing the risk of blood clots. It is classified as a prostacyclin analog and is available in various formulations, including inhalation, intravenous, oral, and subcutaneous administration.
Used to treat pulmonary arterial hypertension (PAH).
Intravenous treprostinil administration may increase the risk of serious bleeding events, particularly in patients with underlying bleeding disorders or those receiving concomitant anticoagulant therapy. Catheter-related infections are also a serious risk and require careful monitoring.
Outcome:
Increased risk of bleeding
Mechanism:
Synergistic effects on coagulation pathways
Outcome:
Potential for reduced effectiveness of treprostinil
Mechanism:
Unknown
Outcome:
Potential for reduced absorption of oral treprostinil
Mechanism:
Changes in stomach pH
Most likely new formulation: Implantable pump delivery system (2025, 80% confidence)
Based on increasing prevalence of PAH and positive clinical trial data, there is a 70% likelihood of expanded indications for treprostinil in the next 5 years.
